Eligible State Health Plan members can enter a drawing to win an iPad if they complete their health assessment. Members who will be at least 18 years old at the time of the drawing and whose primary health insurance is through the State Health Plan are eligible.
To enter the drawing, eligible members must fully complete their health assessment by answering all questions marked with a red asterisk (*) and by providing at least five of the clinical risk measures. These measures include height, weight, waist circumference, blood pressure, cholesterol and blood glucose ("sugar" levels).
Each month, all eligible members who have completed their health assessment on or after Jan. 1, 2011, will be entered into the pool as a potential winner.
The health assessment is available by phone (800-817-7044) and online (www.shpnc.org) through the Personal Health Portal. One promotional iPad drawing will be conducted each month for one year beginning this September for a total of 12 drawings. This is an opportunity for employees to learn their health status and any risk factors, with the added bonus of becoming eligible to win an iPad. The first drawing is set for Sept. 17.
